Back Pain

  • You may experience persistent pain or stiffness anywhere along the spine, starting from the base of the neck to the end of tail bone.
  • Pain is usually sharp and localized experienced in the region of neck, upper back, or lower back — felt especially after lifting of some heavy items or after some strenuous activity.
  • Long term pain in the middle or lower back region, felt especially after sitting or standing for long hours.
  • Back pain may radiate from the low back area to the buttocks, and further down to the back of the thigh, and into the calf and toes.
  • Difficulty in standing straight without experiencing pain or spasms in the lower back muscles.

Back Pain

Mechanical problems

A mechanical problem usually the result of abnormality in the way of spine movement. The most common mechanical cause for causing back pain is supposed to be a condition called intervertebral disc degeneration. This refers to the gradual breakdown of the disc usually located between the vertebrae of the spine with age. With the gradual deterioration of the disc, the vertebrae lose their cushioning ability thereby leading to pain in situations when the back is stressed or sometimes even without stress. Another common mechanical cause of back pain is the wearing down of the facet joints. These are the large joints that connects one vertebrae to the anothervertebra. Other mechanical causes of back pain includesmuscle spasms, ruptured disc or herniated disc, or muscle tension.

TMJD (Temporomandibular Joint Disorder)

This is also considered as an important cause behind back pain. TMJD may arise as a result of-

  • Mal-alignment of teeth or jaw causing the TMJ joint disc to move out from its normal position.
  • Trauma to the jaw causing damage or dislocation of the jaw joint.
  • Inflammatory involvement of jaw joint bone causing damage to jaw joint cartilage.

Acute (short term) or chronic (long term) injuries may be experienced in case of Spine injuries such as sprains and fractures. Sprains occur as a result of tears in the spine supporting ligament due to improper twisting or lifting. Fractured vertebrae are usually the outcome of osteoporosis. Osteoporosis is a condition that causes weak, porous, decreased density bones. Severe injuries like falls and accidents are less commonly found associated with back aches.

Acquired conditions and diseases

Many medical condition and diseases may act as contributory factor to result in back pain in your body like-

  • Scoliosis- results in forward bending of the spine but usually does not result in any back pain until mid-life;
  • Spondylolisthesis,
  • Arthritis-eg-Osteoarthritis, rheumatoid arthritis,
  • Ankylosing spondylitis; and
  • Spinal stenosis (pressure on the spinal cord and nerves due to narrowing of spinal column). Osteoporosis-Though osteoporosis is a condition which itself does not cause any pain but it may result in eventual fracture of the vertebrae resulting in back pain.

Other causes of back pain include- pregnancy, Infection or stone in the kidney, Endometriosis (overgrowth of uterine tissue in sites outside the uterus); and Fibromyalgia may result in easy fatigue and widespread pain in the muscle.

Not a common cause for causing back pain. Infections involving the vertebrae (osteomyelitis) or the intervertebral disc (discitis) may result in some back pain.

Tumors are rarely found associated with back pain. It is rare to find the tumor beginning in the back. Usually a tumor in the back arises as a result of metastasis (spread) from some other site in the body.

Other contributing factors

emotional stress may cause the muscles of back become tense and painful thereby increasing the severity and duration of already existing back pain. In similar manner, anxiety and insufficiently or untreated depression may make the pain of already existing back pain worse. Even lack of sufficient sleep may act as a contributory factor in making your back pain worse.

Spinal Injections

Nerve roots exits from the sides of the spine through small openings. Spinal injections are injected into these openings to reduce the swelling and inflammation of spinal nerve roots and also the tissues present around the spinal nerve root thereby aiding in reduction in pain, numbness, tingling sensation or other symptoms of back pain. Various spinal injections include-

  • Cervical injection
  • Thoracic injection
  • Lumbar injection
  • Interlaminar injection
  • Epidural Injections and
  • Transforaminalinjection.

Stimulation of spinal cord

This procedure involves delivering of low-grade electrical signals to the spinal cord or to the specific nerves so as to block the pain signals from reaching to the brain. Spinal cord stimulation is generally recommended when other treatments modalities have been unsuccessful, or when even the surgery has failed.

Radio-Frequency Ablation (RFA)

Radiofrequency ablation is another procedure carried out to help relieving the pain symptoms of back ache. This procedure involves generation of an electrical current by a radio wave. This electric current generated by radio wave is used to heat up a small specific nerve tissue area, thereby aiding in reduction inpainful signals arisingfrom that specific area. Usually patients with chronic (long term) back pains are treated using this technique. It is generally well-tolerated by the patients and is commonly associated with very less chances of complications. RFA is considered as a safe and effective way of treating few back pain cases.

Ozone Discectomy (Ozonucleolysis)

This is an alternative treatment method in which Ozone-Oxygen mixture is injected into the disc in patients suffering from back and leg pain (sciatica) resulting from disc prolapse. Great number of international researches has confirmedthe efficacy of this treatment. This is considered as a better treatment option than conventional disc surgery.

TMJD Treatment

If the culprit behind your back pain is suspected to be due to TMJD(Temporomandibular joint disorder), then it becomes utmost important to confirm it with the aid of various sophisticated and most advanced diagnostic technology (AVAILABLE ONLY AT Zental Tmj & Body Pain Division  Body Pain Treatment Centre, only SUPERSPECIALIZED DENTAL clinic in INDIA and PERHAPS IN ASIA TOO), like-

Once the diagnosis is confirmed and connection between TMJD and back pain has been established, various treatment procedures can be done to eliminate back aches from the roots. It includes-

  • Home care remedies
  • Medicines-pain relievers, muscle relaxants, anti-depressants.
  • Orthotic/splint
  • Orthodontic treatment
  • Veneers, Crown and bridges
  • Botox

Treating back pain with surgery is always considered as the last treatment option after all other treatment modalities have failed to provide any relief from back pain. There may not be significant improvement experienced by the patient in some cases though. Surgery is generally not found associated with any risks but it may have serious complications sometimes in few patients.
